When/where did throwing coins into a fountain for good luck originate?

A bit of a piggyback question, if I may. Coins are very widely handled and are infamously rife with bacteria. Has the tradition of throwing coins into fountains and wells ever been proven to contribute to the spread of a major epidemic?
