Of all the fast foods, why is pizza the one that's traditionally delivered? When did this first start?

It seems to be one of the few almost universal traits among fast foods - pizza delivery is common and consistent across the board. What's the history behind that, if we know?

I can't answer the question, but I would point out that deli foods, and ethnic foods (chinese, thai, indian) are generally deliverable in urban areas.
