How much did the Latin language change over the course of the Roman Empire?

Medieval-English is quite substantially different than modern English; would the difference between, say, Latin from 200BC and 300AD be noticable?

Yes! There are certain endings and words that are more common in archaic Latin (think Plautus) than the Latin of Cicero. And then medieval Latin is also different with a starting change in pronunciation and the grammatical structure becomes simpler.
