As of today, even the most prominent Polish medievalists cannot answer this question in a definite manner. The most common reasons stated, is relatively low urbanisation (plagues usually strike cities the worst, and spare countryside), being far removed from the Mediterrean where the plague most likely entered Europe, and characteristic for both Slavic and Scandinavian cultures of that time insistence on bathing and cleanliness (debatable).
