I work in military aviation, so accountability is pretty important whenever there's a failure that results in something fatal or could have resulted in something possibly fatal.
Not gonna lie, Apollo 13 (the movie) was on TV and I finally re-watched it. As inaccurate as it may be, it did get me thinking of the incident.
As far as I can gather from reading, it looks like it was a problem of voltage. Space technology in the US was based on 28VDC (like modern US aviation). For some reason, however, it was decided that space technology in the US should switch to 65VDC. It turns out there were some problems with fuel thermostats in that conversion (which, again, as an aviation guy, I immediately understand).
What I can't find, however, is if they ever narrowed it down to one single fuck-up. The best I can find is that the O2 tank may have been dropped, but tested good.
Any fun tidbits, stories, interviews, etc. about Apollo 13 are welcome, as are any point-outs and gripes of movie inaccuracies. /r/AskHistorians seems to be somewhat more lax in rules lately, and I'm all for encouraging it. Your sub-question about inaccuracies in the movie is about 10 days past the 20-year rule, so I'll be bold and answer that one. I hope it's applicable, educational, and enjoyable. The details are from a couple of memoirs from Johnson Space Center/NASA engineers. Also, since your question accused the moderators of being more lax, I really hope this is in-depth enough.
The Mission Control Center at Johnson Space Center in Houston, Texas has multiple Mission Operation Control Rooms (MOCR.) Of note is MOCR 2, which was used for every Apollo mission except for Apollo 7, most of Gemini missions, and was also used for some Shuttle (STS) missions, ending with STS-53 in 1992. After that, the newer consoles were taken out, and the old Apollo 11 consoles were pulled out of storage and reinstalled. It's part of the declared National Registry of Historic Places.
If you ask Ron Howard, he was offered the use of MOCR 2 to film in and declined. If you ask NASA, the cameras wouldn't fit in the aisles without repeatedly removing and replacing historically important consoles so frequently that damage was almost certain. If you ask some engineers, they told the production how long it would take to remove and reinstall a console, and they decided it would be cheaper to just rebuild it. Whatever the truth is there, the film's production entailed making an incredibly exact copy of MOCR 2, down to the very smallest detail.
Here's where it gets fun. NASA has been very big on mission insignia. In every MOCR (or, as they were called for Shuttle missions, Flight Control Rooms (FCR)) a plaque-sized version of the insignia for each mission flown there is installed on the wall. In the case of MOCR 2, as it stands right now, there are badges for every mission run from there, including Apollo 14 through STS-53 in 1992.
Since the movie set reproduction was so thorough and exact, you can catch glimpses of the insignia of later Apollo missions (and some STS missions) in the movie. The Apollo mission insignia you might miss, but in a few parts of the movie, you can clearly see the outline of a Space Shuttle on a few insignia.

If you want further reading on this, Jim Lovell's autobiography (Lost Moon in early editions, lately it's been called Apollo 13) contains a detailed appendix related to the final cause. It was traced to a voltage discrepancy as you surmised. The higher voltage used on the ground damaged some thermal switches which led to the tanks overheating. This damaged the wire insulation which ultimately resulted in a spark and the subsequent explosion.
As with most accidents, it was not the fault of a single person but multiple small failures that led to the accident.
Having met Lovell, he said the film was pretty accurate. The only major difference was that he said the astronauts didn't squabble, they were more professional than that.
Fun fact - Marilyn did drop her wedding ring down the drain in the shower. What they didn't show is that they had a plumber there two hours later ripping apart the bathroom.

As mentioned elsewhere in the thread it wasn't any one persons fault there was a series of small mishaps and mistakes that led to a faulty O2 tank being fitted the Apollo 13 CM
