Do you mean President as a head of state/government or just as a position? English universities had the title as early as the 1400's and in the Commonwealth of England , the Council of State had a Lord President as it's head.

You're right though in suggesting that the President of the USA was the case of the title being used to relate to a single person who was the head of state.
