I wouldn't call it a fake, as it's not retouched or altered in the studio. But it is satirical, and it does appear to be from an unknown movie produced by Thomas Edison. A Snopes forum thread linked to a collections page on the Edison Museum's NPS website that is no longer there, and this blog post gives its accession number (23.430/25) along with the attribution to an Edison film, which suggests to me that it really is a film still. However, I'm not completely sure that I agree that it would have been made in the Black Maria studio, which was only operational between 1893 and 1901: some of the women look to be wearing clothes from about 1900, but the woman in the middle can't be any earlier than 1910, I don't think.
