How are North Korean soldiers motivated?

I've been led to believe that many of those in North Korea know and understand their quality of life is terrible. This leads me to question why anyone in the army actually fights? Surely if conditions are so terrible and their families are starving they would choose to flee or revolt within the military ranks.

Edit: I'll take answers on how soldiers were motivated during the Korean War and if possible current day (to keep this history related).
