So it looks to be a form of trepanation which is a surgery that would have happened in ancient Rome. It has been performed since the neolithic area so a Roman doctor definitely would have known about it. I can't say for sure about the head plate though. The archeological record has uncovered many Roman surgical tools (http://exhibits.hsl.virginia.edu/romansurgical/) but nothing like a metal head plate.
