I think you have to consider Rome was an Aristocratic Republic. Rome was also an empire for over 300 years as well (not counting the Eastern Half). Most of Greeks were not democratic either. Only Athens was a true democracy.
However, the city-state of Florence during the high medieval ages prided itself on Republican values. There were also many merchant republics such as Venice.
