How were Kingdoms determined before lineage?

I know that most Kings/Queens came to power due to their family names/heritage, but there had to be a starting point somewhere. Was there a time that it was like a battle to death or something beforehand? I've always assumed it was by wealth or popularity but would rather have someone more knowledgeable answer.

Hiya, can you please specify a historical context that you're interested in? Medieval Europe will yield different answers than ancient Egypt. AskHistorians requires that threads be focused, to promote extended discussion instead of disjointed individual posts. Thanks! :)
