This is a little overly vague as every country has different rules depending on country of origin. I can go to USA from Canada with passport in hand and no prior approval. I am still not permitted to work, or even solicit orders for my business. If I want to go to somewhere like Sierra Leone I need a visa before I go which can only be given by their consolate or embassy. So there really is no single answer.
