Why did they Black Plague not keep spreading and infecting more humans?

Bubonic plague never really died off. The Black Death was just one particularly nasty attack of it. It's similar to Spanish Flu of 1918 - flu's still around, we just haven't been hit that hard since.

Edit: also, I think this question is more in /r/AskScience territory as the answer is epidemiological/medical, not historical.
