On this point: one book you might be interested in is Justine Siegemund's The Court Midwife, which has an excellent English translation from Lynne Tatlock for the Other Voice in Early Modern Europe series. Siegemund was a midwife in 17th century Germany who worked her way up from a position as village midwife to the local duke's court, to becoming an official member of the court of the Elector of Brandenburg. From there, she traveled to numerous other princely courts throughout the empire. The Court Midwife is her textbook/instruction manual of midwifery based on her experiences.
