Presumably all noble families were started from just a regular person.
I answered a similar question about a week ago, which has a number of links to previous posts about individual families and dynasties. The short answer is that there is no clear-cut, one size fits all answer. European states generally codified the norms of peerage and nobility sometime in the thirteenth and fourteenth centuries, but inequality spanning multiple generations with an upper class hovering above a lower class is a concept that has, quite literally, been around since the stone age.
