To what degree are current sports martial arts "watered down" from their historical roots?

Current sports martial arts have been largely been watered down to make them less lethal/less damaging. For example, eye gouging is banned, despite showing up in both Eastern and Western** street fighting.

In addition, many martial arts organizations have sought to reduce the occurrence of concussions in recent years. Boxing, for example, has sought to eliminate headgear and make boxing gloves thinner, in the theory that you're not going to punch as hard if you are at risk of breaking your hand.

** The relevant statute is #520, making it a felony to gouge or pluck out someone's eye. While not indicative of gouging being part of a formalized fighting system, it does mean that the practice was common enough to be banned.
