Where does the expression "fall on one's sword" come from? Did it actually happen in medieval Europe?

While suicide by sword (e.g. Seppuku) exist in Japan and some other cultures, did literally falling on one's sword actually happen in medieval Europe for disgraced or failed knights or nobles (which I'm assuming the time period the phrase came from)? Was it considered an honorable or acceptable penance? How did it reconcile with the contemporary condemnation of suicide?

I think it originally comes from the Bible - in 1 Chronicles, king Saul is being pursued by his enemies and chooses to fall on his sword.
