How were historic cisterns kept clean? Would there be any issues with mold or contaminants?

Looking at the green stuff on the ceiling of this 1514 Portuguese cistern, the first thing I thought was mold, but it's probably just moss or algae. However, it made me curious about how water quality was maintained before chlorine and other chemical additives.

Could enclosed cisterns such as this have had potential issues with mold, bacteria or other microbial problems? How severe could it get? What about open-air tanks?

Hey, tbh i have no idea but its a really good question. Being myself portuguese, could you tell me where is the image from? Have seen a lot but i never saw this.
