FDR's illness and difficulty walking seem to have been something of an open secret. To what extent was the general American public aware of his infirmity?

The wikipedia entry for FDR's paralytic illness suggests that he went to great lengths to conceal his weakness, but also that the occasional photo was able to slip through the cracks. My question therefore has 3 parts:

  1. To what extent was the average American citizen aware that their president was handicapped?
  2. If they were aware, what was the public's general reaction?
  3. Bonus round: Are we aware of any contemporaneous conspiracy theories relating to his paralysis and its cover-up?
