A question on how to source primary documents from a documentary reader.

Ideally you give as much identifying information as possible, and source it to the reader. E.g., John Smith, "This is an obviously not-real example [1873]," in A Documentary Reader for Examples, ed. Esme Weatherwax (New York: Documentary Publications, 1978), document 243. (The above is Turabian style, "article in an anthology," which is close enough.)
