In classical Athens, were ostracized individuals just banned from the city or were they banned from all of Attica?

All of Attica. In fact, the range extended a bit further than that. A fragment of the Atthis of Philocorus (here but in Greek/Latin only; search for "ostracismo") says that the ostracised person was not allowed to come closer than the promontory of Euboea. I can't find any other geographical descriptions, however.
