Did people of Middle Ages Europe intentionally throw their excrement into rivers (directly or indirectly)? unintentionally? Or did they dispose of it elsewhere?

All I can find are articles and questions on whether they through it out the window, and it's quite annoying.

By directly intentional I mean throwing their chamber pots by hand, by indirectly intentional I mean they knew that their sewers were dumped upriver or what have you, by unintentional I mean that their containment didn't work how it was supposed to, and the last one is self explanatory. Thanks!
